Hospice Giving Foundation leads various initiatives to improve end of life care. Through close collaborations with local providers and feedback from community members, we stay abreast of emerging issues and develop response strategies to improve the quality of care, expand access to care, advance knowledge, and promote preparedness.
HOPEGives
We understand. Death is a difficult, painful part of life. That’s why Hospice Giving Foundation leads HOPEGives, a movement to shift societal attitudes about death. HOPEGives invites you to view end of life as a precious time to support, encourage, and care for one another. HG Foundation is here to help change the conversation about death and dying and to foster acceptance for this universal human experience. By deepening understanding, HOPEGives helps individuals and families experience peace of mind through advance planning and compassionate end-of-life care
HEAL TOGETHER
In early April 2021 the Board of Directors for Hospice Giving Foundation established a major grant initiative called Heal Together, to support the expansion of grief and bereavement services throughout Monterey County. Heal Together will fund the coordination of bilingual/bicultural services and a wide range of interventions, structured so it is easy for people to get the help they need. Providers will have funding to expand services and individuals would get care regardless of whether they have a means to pay.
EQUITY + ACCESS
IN END-OF-LIFE CARE
Recent years have shone a light on the lack of equity in healthcare, as well as in many other realms of our society. In order to effect change, we must begin by acknowledge the disparities. Care is not the same, nor is it available, for everyone. Meaningful change requires thoughtful examination of factors like race, culture, socioeconomics, and long-standing, historical behaviors and influences. It also requires listening to those who have been personally impacted and giving them a voice to state what they need, want, and expect. Guided by people’s life experiences, our collaborative work with providers and service partners allows Hospice Giving Foundation to support systemic change. PROMOTING PREPAREDNESS
We want you to be prepared and we’re here to encourage and help. HG Foundation’s initiative to increase the number of people with a completed Advance Health Care Directive is a priority. Now is the time to plan for end of life. Being prepared lets you live life more fully and with peace of mind. JUNTOS CON ESPERANZA
Juntos con Esperanza is a partnership to promote access to quality care and services through end-of-life for people in Latinx communities. Our focus is to better understand desired/needed services, the influence of community members’ culture, faith, and family relationships, and how to make culturally relevant care, education, and services available.
Facilitated by HG Foundation, this partnership has broad representation from community organizations: Local churches and school districts, Alliance on Aging, Central Coast VNA & Hospice, Hospice of the Central Coast, Legal Services for Seniors, Mee Memorial Hospital, Monterey Department of Social Services, and Natividad Medical Center.
MONTEREY COUNTY PALLIATIVE CARE COLLABORATIVE
The Monterey County Palliative Care Collaborative, formed in 2018, is a monthly forum for clinician-leaders in palliative and hospice care of Monterey and San Benito Counties. We seek to foster and facilitate communication and sharing of ideas, talents, skills and energy, with the aim of improving the coordination and delivery of palliative and hospice care services locally and aim to transcend institutional boundaries to achieve this common goal.
