COLLABORATIVES
Hospice Giving Foundation leads and supports collaborative initiatives that strengthen end-of-life care across our community. By bringing together local providers, caregivers, and community voices, we create space to listen, learn, and respond to the real needs people face at the end of life. Through these close partnerships and ongoing community feedback, we stay attuned to emerging issues and evolving gaps in care. This collaborative approach allows us to develop thoughtful response strategies that improve quality of care, expand equitable access to services, advance shared knowledge, and promote preparedness for individuals, families, and the professionals who support them.
The Monterey County Palliative Care Collaborative, formed in 2018, is a monthly forum for clinician-leaders in palliative and hospice care of Monterey and San Benito Counties. We seek to foster and facilitate communication and sharing of ideas, talents, skills and energy, with the aim of improving the coordination and delivery of palliative and hospice care services locally and aim to transcend institutional boundaries to achieve this common goal.
Juntos con Esperanza is a partnership to promote access to quality care and services through end-of-life for people in Latinx communities. Our focus is to better understand desired/needed services, the influence of community members’ culture, faith, and family relationships, and how to make culturally relevant care, education, and services available.
Facilitated by HG Foundation, this collaboration has broad representation from community organizations: Alzheimer’s Association, Central Coast VNA & Hospice, Coastal Kids Home Care, Health Projects Center, Hospice of Santa Cruz County, Montage Health, and Legal Services for Seniors.
