GRANTS IN ACTION

End-of-life care does not happen in isolation. It unfolds in hospital rooms, family homes, and community spaces, and it requires collaboration, trust, and compassion at every step. Hospice Giving Foundation exists to help knit these efforts together, ensuring individuals and families receive care that is coordinated, responsive, and rooted in dignity. Through our annual grantmaking, Hospice Giving Foundation invests in organizations that provide a continuum of care across Monterey, San Benito, and southern Santa Cruz Counties. In our 2025 grant cycle, we awarded $1,135,000 to 12 community partners supporting hospice and palliative care, pediatric services, caregiver support, grief care, spiritual care, and access to basic needs. Our funded partners strengthen care across settings and communities, responding to the diverse and complex needs people face at the end of life. Together, we are building a system of care that meets people where they are—when stability, compassion, and human connection matter most.

Stories of Impact

“The support that Hospice Giving Foundation provides to our patients and our entire community is invaluable during what is arguably one of the most difficult times a family will face. When grieving a loss, the added burden and stress surrounding even the most basic of final expenses can be devastating. We are so grateful to Hospice Giving Foundation for providing support and partnering with us to provide quality care in our community.”
Salinas Valley Health Foundation

“The compassionate care made an immeasurable difference during her final days.”

When her mother, Darlene Acosta, was diagnosed with advanced cancer, Jerry’s Place provided not only medical care, but comfort, dignity, and peace for both her and her family.

Looking Ahead

Together, we are creating a community where no one walks through illness, caregiving, or grief alone.
