WORKSHOPS

This advance care planning workshop is open to all of those who would like assistance in completing an Advance Health Care Directive, POLST form, letters to loved ones, writing your own obituary, planning your own funeral/celebration of life, and using Notes to My Family in order to feel ‘fulfilled’ in your end-of-life plans.

Facilitators: Shary Farr and Fred Jealous will also explore the emotional and spiritual experiences which making these plans may create in all of us.
